Everyone has probably visited the dentist at least once. The reason is likely due to cavities. As children, we may have cried while visiting the dentist because teeth weren’t brushed properly due to inexperience, and many adults have experienced similar situations. Additionally, some visit the dentist regularly for preventive care as a result of neglecting dental health. Teeth serve a purpose far beyond simply chewing food; they significantly impact our speech, facial structure, and even overall health. Therefore, the importance of dental care cannot be overstated. In this article, I will guide you through the treatment methods for cavities—that very condition many have experienced and suffered from.
First, let’s understand exactly what a cavity is. Cavities occur when the surface of the tooth crown, called enamel, is damaged by acids produced when sugars and starches break down. When cavities are confined to the enamel, they cause little to no pain. However, if they progress beyond the dentin into the pulp, pain can be felt even without any stimulus, and painkillers become ineffective. If left untreated, the decay progresses, requiring increasingly larger treatment areas. Therefore, it is crucial to consult a dentist promptly to find the appropriate treatment for the patient.
Early symptoms of cavities are often very subtle and difficult to notice on one’s own. Patients frequently remain unaware of a cavity until pain occurs, often leading to delayed treatment. Regular dental check-ups are essential to avoid more complex and painful treatments at this stage. The reason early intervention for cavity prevention is so crucial is that teeth cannot naturally repair themselves once damaged. Once a cavity forms, preventing its spread becomes the priority. Therefore, cavity treatment focuses on removing the decayed area and filling it with an appropriate dental material. Since each material has its own advantages and disadvantages, it is important to fully understand these before deciding on the treatment approach.
First, amalgam fillings, while raising concerns about mercury-related safety, are an effective treatment meeting U.S. Food and Drug Administration (FDA) standards. They are inexpensive, easy to work with, and require no adhesive. However, the silver-gray color of amalgam does not match natural tooth color, appearing unnatural, and its setting time is very long at 24 hours. Therefore, chewing on the treated side must be avoided for a full day after treatment. Another disadvantage is that more tooth structure must be removed compared to other materials to ensure retention.
Another treatment option is composite resin filling. This method involves etching the tooth surface with acid to bond the resin. During this procedure, the resin cures completely during treatment, making it convenient to manage. It withstands chewing forces well, posing little risk of fracture. Being a smaller filling material, it requires less tooth reduction than amalgam. Therefore, it is suitable for conservative treatment. Its aesthetic quality is also good. However, resin discoloration may occur over time, so it is advisable to avoid pigmented foods like coffee, chocolate, and curry. Additionally, resin shrinkage after treatment may potentially cause pain.
Dental treatment is important not only for physical care but also for providing psychological comfort. Many people fear the dentist not only because of the pain during treatment but also because the experience itself causes anxiety. Therefore, a gentle approach considering the patient’s psychological state and thorough explanations are necessary, helping the patient feel more comfortable during treatment. Glass ionomer cement procedures are primarily used for cervical abrasion. Abrasion, also known as cervical tooth abrasion, refers to the wear of the exposed tooth surface where the gum recedes at the root area, meaning the cementum layer and underlying dentin are exposed due to gum recession. Unlike resin, it does not require acid etching of the tooth. The material fully cures only after 24 hours and has the drawback of low strength, making it susceptible to chewing forces and prone to fracture. However, it offers a natural appearance and minimal pulp irritation. Additionally, it releases fluoride, providing a protective effect against secondary caries in patients prone to cavities.
The filling materials introduced earlier were used for relatively small areas, essentially for filling cavities. In contrast, the inlay procedure described now is chosen when the damage is severe enough that filling is insufficient, and a tooth needs to be reconstructed entirely. An inlay procedure involves attaching a custom-made inlay, fabricated at a dental laboratory, to the tooth using dental cement. Unlike other procedures, inlay treatment requires two visits: one to create the tooth model and another to attach the fabricated inlay. Between these two visits, a temporary filling is placed. Caution is needed during this period, as sticky foods can cause the temporary filling to dislodge. There is also a possibility of dislodgement after the procedure. Inlays can be broadly categorized into three types based on the filling material. First, gold inlays have a physical composition most similar to teeth, allowing for relatively less tooth reduction. While they pose no risk of breaking, they do not match tooth color and thus have poor aesthetics. Resin inlays offer good aesthetics but have a higher risk of damage or discoloration compared to gold inlays. Finally, ceramic inlays also offer excellent aesthetics. Their strength exceeds that of tooth enamel, which may cause wear on the opposing teeth.
Cavities do not heal naturally. Of course, if you maintain oral hygiene by brushing more frequently or using floss after a cavity develops, the decay may halt, reaching a state of arrested caries. However, the tooth cannot return to its original state. Therefore, striving to prevent cavities is the most desirable approach. The most effective way to maintain dental health is prevention, which requires consistent care. Beyond brushing and flossing, dietary habits also play a crucial role in cavity prevention. Habits like frequent consumption of sugar or sugary drinks can cause cavities and should be avoided. Additionally, regular dental check-ups and professional cleanings are recommended. Pay attention to the condition of your teeth, and if a cavity is developing, consult thoroughly with your dentist to receive appropriate treatment based on your health, the cause of the decay, and its progression. Severe toothaches not only prevent us from properly enjoying the foods we crave but also become a significant source of stress when they disrupt daily life.
